Prolactine kan voorkomen onder verschillende vormen: monomeer prolactine, dimeer prolactine en een polymere vorm, het macroprolactine. Deze polymere vorm heeft geen biologische activiteit.
Sporadisch kunnen IgG auto-antilichamen tegen prolactine binden aan prolactine en resulteren in de vorming van macroprolactine.
Macroprolactine heeft een lagere klaring dan prolactine wat kan resulteren in een verhoogde totale prolactine concentratie in de afwezigheid van verhoogde prolactinesecretie door de adenohypofyse welk kan leiden tot een incorrecte diagnose van hyperprolactinemie.
Omdat macroprolactine ook gemeten wordt met de routine prolactine assays kan de aanwezigheid van macroprolactine uitgesloten worden door toevoeging van polyethyleenglycol (PEG) aan het serum, welk zorgt voor de precipitatie van het macroprolactine. Macroprolactine is aanwezig als de recovery van prolactine na PEG precipitatie minde dan 40% bedraagt.
